I saw one person say they broke vengeance oath by not killing the evil hag. Vengeance oath is to kill evil things. Try not killing something evil you encounter. Or you could respec to Oath of devotion and kill one random person who's not evil. If you do the quest involving and , you are given the choice to torture or forgive the latter.
